We are not part of, affiliated with, contracted by, or endorsed by the NHS, NHS England, NHS Improvement, any individual NHS trust, or the Department of Health and Social Care. References to the NHS throughout this site describe the maternity care system this tool is designed to support, and reflect general NHS practice in England and Wales — they do not imply any official relationship.

1. What this site is

My Birth Plan UK is a free questionnaire-based tool that helps you organise your thoughts and preferences for labour and birth into a structured document, intended to support conversations with your NHS midwife or maternity team. It is designed with reference to general NHS maternity practice in England and Wales, but maternity care varies between NHS trusts, hospitals, and individual circumstances.

2. Reviewed by practising midwives

What this means The content and structure of this tool has been reviewed by practising midwives for general clarity, accuracy, and relevance to NHS maternity care. This review reflects feedback at a point in time and does not constitute ongoing clinical supervision, endorsement of any individual user's specific birth plan, or a guarantee that the content reflects the current policy of any particular NHS trust. It is not a substitute for advice from your own named midwife or care team, who are best placed to advise on your individual situation.

3. No doctor-patient or midwife-patient relationship

Using this site does not create a clinical relationship between you and My Birth Plan UK, its creators, or any midwife who has reviewed the tool's content. We do not provide individualised medical advice, diagnosis, or treatment recommendations of any kind.

4. Accuracy and currency of information

Maternity guidelines, NHS trust policies, and clinical best practice can and do change. While we aim to keep general references (such as NICE guidance mentioned within the tool) reasonably current, we cannot guarantee that all content is up to date, complete, or applicable to your specific NHS trust or hospital. You should confirm any clinical details, options, or trust-specific policies directly with your midwife or maternity unit.

5. Your birth plan is a starting point for discussion

A birth plan generated using this tool reflects preferences you have entered and is intended to support, not replace, ongoing discussion with your maternity team. Birth is unpredictable, and clinical decisions may need to depart from any written plan to protect the safety of you or your baby. Nothing in this tool, or in any document it generates, overrides the clinical judgement of your attending healthcare professionals in an emergency or otherwise.

6. No liability

To the fullest extent permitted by law, My Birth Plan UK, its creators, and any contributors (including reviewing midwives) accept no liability for any loss, harm, or damage arising from your use of this site or reliance on any document it produces. This includes, without limitation, any consequence of decisions made during labour or birth, whether or not informed by a plan created using this tool.

7. Data loss

As explained in our Privacy Policy, this tool does not save or store your answers. If you close the page, refresh, or experience a technical issue before downloading or printing your completed plan, your answers cannot be recovered. We are not responsible for any loss of your in-progress birth plan.

8. Intellectual property

The design, structure, and content of My Birth Plan UK are owned by us or used with permission. You're welcome to use the tool to create your own personal birth plan and to print or share that document freely. Please don't copy, republish, or redistribute the tool itself or its underlying content without permission.

9. Changes to the service

We may update, suspend, or discontinue any part of this site at any time without notice.

10. Governing law

These terms are governed by the laws of England and Wales.
